Open a review and the scores are already there.

Reviews start from the scores recorded through the period, and an improvement plan links back to the review that raised it.

Let people score themselves before the manager does.

The same targets are scored twice, once by the person and once by their manager, each with their own notes on every target. The two sit next to each other, so the meeting starts where they disagree. Finalising the review locks it, with the date it was signed off.

Goals

Agree a goal, approve it, and watch it move.

A goal has a title, a date it is due by and, if it helps, the target it feeds. It starts as a draft for the manager to approve or turn down. After that it carries a percentage both of you can see, and it closes itself when it reaches a hundred.

The rating

Get a rating everybody reads the same way.

Each target is measured against its own goal and capped there, so beating one does not cover a missed one. The weights turn those into a single number out of a hundred, and the number falls into one of five bands. Nobody types the rating in, so the same score always reads the same way.

Who can do what.

Everyone can see their own targets and the scores recorded against them. Reviewing somebody else, and defining the KPIs everybody is measured on, are separate permissions.

Admin is the highest role and is not limited by these permissions, so give it only to the small group of people who truly need it.

One cycle, start to finish

Targets on day one, a review six months later.

The KPI template came with the role, scores were recorded as the work happened, and the review opened with those numbers already in it. The manager's job was to add judgement on top. The previous cycle stays on the record, so the next review has something to compare against.
